SMILE Canada supports children and youth with disabilities, along with their families, across Canada. To register, the child must:
1. Have a disability and/or long-term health condition (no formal diagnosis required),
2. Be under 30 years old, and
3.Live in Canada.
Our priority is to support families from specific ethno-cultural backgrounds who need help navigating services in Urdu, Arabic, Farsi, Dari, or Somali. Requests from families outside these priority groups are reviewed individually, based on our capacity and operational needs.
Complete an intake form. The intake form can be found in the following languages: English, Arabic, Farsi and Somali
The eligibility committee reviews the intake. If the child meets the eligibility criteria (as stated on the intake forms), a Service Navigator is assigned based on the family’s preferred language.
The Service Navigator reaches out to the family to schedule a needs assessment typically at the 4 - 8 week period after the intake form was submitted.
Please note: The Service Navigator who completes the needs assessment with the family will be their assigned Service Navigator, unless otherwise communicated.
Once the needs assessment is completed, a care plan is created for the child or youth. The Service Navigator then supports the family by identifying appropriate resources (internally or externally) and/or enrolling the parent or child into programs based on the identified needs within 2 weeks.
The Service Navigator checks in via email and phone every 3 months with the family to ensure goals are being met.
