Join our free virtual session to learn how to support your youth as they prepare for adulthood. As children and youth grow older, their needs and the support available to them can change. This session will help families better understand what to expect and how to prepare for the next stage of their youth’s life.
We have three speakers lined up for this event:
Halla Fahmi is a registered social worker with the bridging to adulthood team at holland bloorview kids rehabilitation hospital. She supports youth with disabilities and their families through the transition to adulthood
Andrew Lauzon is a nurse practitioner with the LIFEspan transition team at holland bloorview kids rehabilitation hospital and Toronto reha, with over 30 years of nursing experience. She has supported youth with childhood onset disabilities as they move from paediatric to adult health services.
Nadia Niles-Campbell is an occupational therapist at holland bloorview kids rehabilitation hospital with 20 years of experience helping youth with disabilities plan and prepare for the move from highschool to post secondary education.
This session is open to parents, caregivers, and family members of children and youth with disabilities living in Ontario.
Whether you’re just starting to think about the future or already planning for the transition to adulthood, this is an opportunity to connect with other families and explore the supports available to your youth.
